Your Privacy Matters

Nourished Living is committed to protecting your personal information and ensuring your data is handled safely, respectfully, and in accordance with the Australian Privacy Principles (APPs) and the Dietitians Australia Code of Conduct. This page explains how your information is collected, stored, used, and protected when you engage with Nourished Living. 

Information Collected

To provide safe, effective nutrition care, Nourished Living services may collect:

  • Your name, contact details and date of birth
  • Medical history, medications, allergies and relevant clinical information
  • NDIS plan details (if applicable)
  • Appointment notes and progress records
  • Payment information and billing details
  • Any documents you choose to share (e.g. reports, letters, growth charts)

This information is collected directly from you, your authorised representative or - with your consent - from other health professionals involved in your care.

How Your Information Is Used

Your information is used to:

  • Provide safe, evidence‑based nutrition care
  • Tailor recommendations to your needs
  • Communicate with you about appointments and services
  • Coordinate care with other health professionals (with consent)
  • Meet legal, clinical and NDIS documentation requirements
  • Process payments and invoices

Nourished Living will never sell, trade or disclose your information for marketing purposes.

How Your Information Is Stored

Your information is stored securely using password‑protected systems and encrypted digital records. Paper documents (if used) are kept in locked storage.

I take all reasonable and necessary steps to protect your information from:

  • Loss
  • Misuse
  • Unauthorised access
  • Modification
  • Disclosure

Records are retained in accordance with Australian health record legislation.

Sharing Information

Your information may be shared only when:

  • You provide written or verbal consent
  • It is required for coordinated care (e.g. GP, paediatrician, allied health team)
  • It is required by law (e.g. court order, mandatory reporting)
  • It is necessary to prevent a serious threat to your health or safety

You may withdraw consent at any time.

Accessing Your Information

You have the right to:

  • Request access to your personal information
  • Ask for corrections if information is inaccurate or incomplete

Requests can be made via the Contact page. Identification may be required.

Confidentiality

All sessions and personal information - in‑clinic, in‑home or via telehealth - are confidential. Information is only shared with your consent or when legally required. 

Terms and Conditions

Appointments

By booking an appointment, you agree to:

  • Provide accurate information relevant to your care
  • Attend sessions on time
  • Notify me promptly if you need to reschedule
  • Pay fees as outlined on the Pricing page

Telehealth clients are responsible for ensuring a private, safe environment for sessions they attend.

Cancellations

To ensure fairness and availability:

  • Cancellations with more than 24 hours’ notice incur no fee
  • Cancellations within 24 hours may incur the full session fee
  • NDIS cancellations follow the current NDIS cancellation policy

If you are unwell or experiencing unexpected circumstances, please reach out - flexibility is always considered.

NDIS Responsibilities

NDIS participants (self‑managed or plan‑managed) agree to:

  • Provide accurate plan details
  • Ensure funding is available for booked services
  • Notify Nourished Living of any changes to plan dates or management
  • Pay invoices within the required timeframe (self‑managed)

Nourished Living adheres to the NDIS Code of Conduct in providing safe, respectful, person‑centred care.

Clinical Scope

Nourished Living provides evidence‑based nutrition care within the scope of practice defined by:

  • Dietitians Australia
  • NDIS Practice Standards
  • Australian health legislation

Nourished Living does not diagnose medical conditions or provide services outside of a Dietitian's professional scope. If an issue requires another clinician, Nourished Living will recommend appropriate referral pathways.

Nourished Living (Dietitian) Responsibilities

Nourished Living is committed to:

  • Providing safe, respectful, evidence‑based care
  • Maintaining confidentiality and privacy
  • Acting in accordance with the Dietitians Australia Code of Conduct
  • Ensuring culturally safe and inclusive practice
  • Communicating clearly and professionally
  • Prioritising your wellbeing and autonomy
  • Keeping accurate, secure clinical records

Your Responsibilities

To ensure safe, effective care, clients agree to:

  • Provide honest, accurate information
  • Inform your Dietitian of any changes to health status, medications, or support needs
  • Follow safety instructions during in‑home or practical sessions
  • Ask questions if anything is unclear
  • Communicate respectfully and appropriately

Website Use

By using this website, you agree not to:

  • Copy or misuse content
  • Attempt to breach security or access restricted areas
  • Use the website for unlawful or harmful purposes
